We are seeking a dedicated Geriatrics Physician to provide locum tenens coverage for our clinic and skilled nursing facility (SNF) in Vermont. In this role, you will primarily focus on caring for geriatric patients, including preventative visits, addressing acute medical needs, and managing chronic ongoing conditions. Your responsibilities will also include SNF admissions, discharges, acute care, and periodic assessments. Key Responsibilities: Provide comprehensive medical care to geriatric patients in both clinic and SNF settings. Perform preventative health assessments and screenings for geriatric patients. Address acute medical needs and manage chronic ongoing conditions. Manage SNF admissions, discharges, acute care, and periodic assessments. Collaborate with a supportive healthcare team to ensure the well-being of elderly patients. Conduct direct patient care with a focus on patient comfort and satisfaction. Utilize electronic health records (EHR) for documentation and patient management. No procedures are required in this role. Qualifications: Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree. Board certification or eligibility in Geriatric Medicine is preferred (Will consider Family Medicine or Internal Medicine) Current medical license in the state of Vermont. Strong commitment to providing high-quality care to geriatric patients. Excellent communication and interpersonal skills. COVID-19 vaccination is required for this position. Schedule: Locum tenens position starting on December 11th. Monday to Friday, 8:00 a.m. to 5:00 p.m. Approximately 1.5-2 days in the clinic and 3-3.5 days in the skilled nursing home. Patient load: Up to 16 patients in an 8-hour clinic day and 8-12 patients per day in the SNF. Use of PCC and EPIC electronic health records in the SNF, and EPIC only in the clinic.
